PADI Medical Form
This Form Must be filled out for every PADI course taken with IDC.
One of the questions we receive at IDC is do need to see a doctor.
The answer is NO if you when you are filling out the Medical form and answer NO to all the questions. You must answer all of these questions honestly and if you are unsure then it is probably a good idea to consult a physician*.
If you do answer YES to any of the questions this does not mean you are unable to take a course with us. It simple means you need to print out the PADI Medical statement in full and take it to a doctor.
*Sorry IDC staff cannot give out medical advise. We are not doctors, but we have wanted to play them on TV.
