PADI MEDICAL STATEMENT: This
form is used for screening any medical conditions that may be a potential
problem for the student diver. Every person wishing to participate in any
scuba course must complete the PADI Medical Statement prior to any training.
As you read the first page of the form, you will notice a series of questions
about your current medical state and past medical history. As you go through
all of these questions, you must answer “YES” or “NO” to
each question. We ask that you read the form thoroughly, make sure you understand
it and try to answer the questions honestly.
If you are able to go through all of the questions and answer “NO” to
each and every one, then you need not proceed any further with the Medical Statement
at this time. On the first night of your training at the class orientation and
paperwork portion, you will be asked to fill in the form and can answer “NO” to
all of the questions at that time. Being that your form is all “No”,
you do not need a doctor’s clearance in order to participate in the Scuba
class.
If you go through all of the questions and find that you must answer “YES” to
one or more questions, then you must do the following:
- Print out the PADI Medical Statement in full (6 pages) and take it to
your family physician or to a clinic if you do not have a family physician
in town. Have the physician discuss the conditions to which you have answered “YES” and
decide with you if Scuba training is safe and appropriate for you.
- Have the physician complete PAGE 2 of the form in full and check the
box toward the bottom of the form that does or does not give you medical
clearance to do scuba training. Have the physician sign and stamp PAGE
2 of the form as well.
- Bring the signed form to the staff at International Diving Centre on
session 1 of your training.
IMPORTANT*******
If you have not received medical clearance from a physician and have answered “YES” to
one or more questions on the medical form, you will be unable to participate
in any in-water training for any scuba course at International Diving Centre.
This is true for all PADI diving facilities across the globe.
A "YES" to a question doesn't typically stop a person from participating
in scuba lessons when the medical condition is mild. It is however important
to receive medical clearance as some conditions can become very serious
when the body is subjected to increased pressure. With this in mind, it
is important to answer honestly and seek medical advice where required.
The staff at International Diving Centre are not physicians and accordingly
are unable to give medical advice of any kind. Should you have questions
